Apply for service
To apply for services through New Jersey’s Division of Developmental Disabilities (DDD), you must be at least 18 years old, live in New Jersey, and have a developmental disability that began before age 22. You also need to be eligible for Medicaid. To start, complete the DDD intake application, which you can download from the DDD website or request by phone. Along with the application, you’ll need to send documents that show proof of disability and Medicaid. After reviewing your information, DDD will let you know if you’re approved.
