The Division is able to support more than 25,000 people with services because the federal government helps cover the cost through funding for the Community Care Program (CCP) and Supports Program (SP). To receive this funding, individuals, providers, and the services they use must follow all Medicaid rules at both the state and federal levels. That’s why it’s necessary for anyone enrolled in CCP or SP to keep their Medicaid coverage active.
Since there are several ways to apply for Medicaid, it’s important to know which option is right for you.
If you currently receive—or are trying to get—DDD services and your Medicaid is denied or stopped, you should complete the DDD Medicaid Eligibility Troubleshooting Form. Send the form and a copy of your Medicaid denial notice to the DDD Medicaid Eligibility Helpdesk by email at [email protected] or mail it to:
