Working Together to Support People and Their Choices
The NJ DDD works closely with individuals, families, and service providers to make sure people are healthy and safe, while also respecting their right to make their own choices. In the community, people can get many different services through a network of over 500 providers. These services are planned with the help of independent Support Coordinators and may include assistive technology, behavior supports, day programs, job opportunities, therapy, transportation, and the option to choose and direct your own staff and supports—including taking classes in the community.
People receive these services through one of two Medicaid waiver programs:
Trifecta Home Care is one of the NJ DDD Support Providers and we offer the following services in the counties listed below.

People receive these services through one of two Medicaid waiver programs:
- Community Care Program (CCP): Started in 1985, this program gives people an option to live in the community instead of in institutions. Over 11,000 people now use CCP to live in group homes or receive full supports at home.
- Supports Program (SP): This program began in 2016 to help more people—about 11,000—get important services and supports while living at home with their families.
