Support Coordination Agencies and their coordinators provide independent case management for individuals who receive DDD-funded services. They collaborate with the individual and their planning team to develop and update the person’s yearly individualized service plan as needed.
Support coordinators meet with the individual at least once a month, either face-to-face or through phone or video calls, to monitor their well-being and safety and confirm that the services in their plan still fit their needs.
