Companion CAre
Recent studies estimate that approximately 65 million people in the United States (1 in 5 people) feel lonely. Another study found that 12% of Americans have no one with whom to spend free time or to discuss important matters. These statistics are even more grim for the elderly and infirm. Loneliness has been aptly described as social pain—a psychological mechanism which motivates individuals to seek social connections because of its effect on our emotional wellbeing (more information here). The feelings of loneliness can be amplified if you are no longer capable of independently moving around or carrying around easy household tasks and chores. This is where our caring and trained caregivers come in handy. 

Our caregivers do not just spend time with our clients just to while time away. We take time to learn about the things and activities that our clients enjoy and incorporate them into the activities they will partake with members of our team. We not only seek to maintain their quality of life. We want to improve it by being true companions who will also partake in the following activities with our clients:
  1. Transportation for errands or relaxation.
  2. Playing on the computer / tablet or reading a book/newspaper out loud.
  3. Playing games
  4. Going for a walk around the neighborhood or at the park
  5. Other chores like responding to emails, paying bills, etc.

Homemaking can be challenging for many people, regardless of age and physical fitness. Our team is well equipped to help our clients with any homemaking activities, including, but not limited to
  1. House cleaning, including vacuuming and dusting
  2. Cooking and dishes
  3. Laundry 
  4. Organizing the home
  5. Watering indoor plants
  6. Feeding pets 
 ​​The cognitive benefits of socializing and spending time with other people are well documented, but there are other benefits that come from the companionship provided by our team. For one, the presence of another person provides the added safety of being guided through difficult tasks or just having supervision.  There is also increased mobility from being driven or wheeled around places that one cannot go by themselves. 
HOSPICE CARE
Making a decision about hospice care can be a challenge.  We recently found this quiz to be very helpful in making that decision for a friend. The sure way that most everyone knows is that you can elect to enter a program if your doctor states that you have a terminal illness, and death will likely happen in 6 months or less. What happens if you enter and you do not pass away? You can stay in hospice beyond those six months if your doctor states that you still have a short time to live. Just because one choses hospice does not mean they have given up or lost the will to live. You can change your mind and get back to other treatments even after having been in hospice for a few months. Sometimes people get better, albeit unexpectedly, and they deserve the right to change their minds. So how does Hospice differ from palliative care? Palliative care includes everyone who is seriously ill, not just those who are facing imminent death or are no longer interested in treatment.  Hospice care is covered by Medicare for all eligible patients and by most Medicaid and commercial insurance plans. 

In most states, including New Jersey and Pennsylvania, Hospice offers four levels of care. Two of them happen at home, and our team is trained to help with the first of the four levels described below. 
  • Routine Home Care. This is the most common level of hospice care, and it includes includes nursing and home health aide services.
  • Continuous Home Care. This entails continuous nursing care at home. 
  • General Inpatient Care. Short-term care during those times when pain or symptoms can’t be managed outside a hospital setting.
  • Respite Care. Short-term care in a facility during times when caregivers need a break.

How does home care with trifecta work?


We provide the best and most personalized and compassionate Home Care for your loved ones.
  • WE START by consulting with and planning for care for the client and their family via a home visit or telephone call.
  • THEN we arrange for a free assessment of needs by our nurse supervisor, who then develops a plan of care with the family.
  • NEXT we manage all aspects of the care that you entrust to us by mobilizing human and material resources to meet your goals. Our well-trained, and certified caregivers work under the supervision of the nurse supervisor to provide care.
  • FINALLY, we regularly (monthly phone calls and 60 day in person visits) assess our effectiveness in meeting our shared goals and adjust care as needed.​

Below are four steps detailing how we strive to produce the results you desire!

The FIRST CALL AND FIRST MEETING
When you first contact us, either via email or phone, we will start working with you to gather as much information we need to provide the best possible care for your loved one. By the time we set-up our first in-home visit with you and your family, we would have gathered enough information that will better help us evaluate your needs as well as how we can better meet them. 
COMING UP WITH A PLAN OF ACTION - ONE OR TWO HOME VISITS
During our first meeting with you, we will get acquainted with you in person (or via Zoom if the Covid-19 Pandemic persists) and find out more about your loved one, basing our questions from our initial call or email exchange. You will also get a chance to see who we are as an agency. During this time, we will seek to know about any special needs your loved one may have and note any specific recommendations from their healthcare providers or previous caregivers.  We will also learn a bit about the type of person they are so that we can come up with the best possible care plan for them. Not every person is the same.

After this meeting, we will send our notes to our nurse supervisor, who will arrange for a home visit to do a formal assessment of needs. After this visit, she will develop a specific plan form your loved one based not just on their needs, but also on who they are as a person and their specific environment. This is the plan of care on which our services will be based and it will also include your input and our recommendations.

This is a dynamic plan that changes as your needs change, and we will make sure that you are satisfied with every part our plan and that it is tailormade for your loved one.
matching you with the best caregiver for your loved one
Every client is different. So after you approve of our proposed care plan, we will match that plan with the caregiver who best fits the requirements of that plan. We do this through both technology (software) and our own intuition about the caregivers on our team. We guarantee that, if you have any concerns about any caregiver we propose for our plan, we will find a suitable replacement who fits better with your particular situation.

For your peace of mind, every caregiver on our team is a W-2 employee, and is covered by our liability insurance and workman's compensation (in case they get injured in your home). As an agency, we are also insured and bonded against thefts and damage from accidents in the home.
implementation and Assessment
In order to keep track of the progress we are making towards our caregiving goals, all our caregivers are trained to take very detailed notes about their activities and those notes are kept at the point of service (your home). In an effort to stay transparent, you can request to see those notes at any time and compare them with what your loved one says or what you see in the home. Our managers also pay unannounced visits to the homes of our clients to ensure that the quality of care given to our clients is the highest quality that meets Trifecta Standards.  The invoices you receive will also detail the services provided by the caregivers (in other words, you will not be billed for services not rendered). 

Trifecta uses technology to insure that when caregivers clock in for work, they are at the location that corresponds to your address. We also require that they sign-off the specific activities they carried out during any shift and give you the opportunity to sign off on them. This allows for accountability and opens up doors for open communication if certain needs are not being met.

Three essential questions to ask to ensure the company you choose has a strong set of core values:

  • Communication – Will I be able to easily communicate with the caregiver and have continued access to my case manager? This is the core of our approach. We believe that you should never feel there is a lack of control or communication with your caregiver. For this reason, you are assigned a case manager who is always available. Silviya and nurse supervisor speak directly with you every month, and the RN stops by every 60 days.
  • Integrity – Has a sense of trust been established? We believe this is the key to a comfortable relationship and the well-being of the client. That’s why we carefully screen all our caregivers and address any compatibility issues swiftly.
  • Performance – Am I confident I will get the professional care promised and that there will be accountability and follow-up? Home care is dynamic; a family’s needs can change daily. You chose home care to allow your loved one to remain independent and retain their quality of life, not to endure more paperwork and problem-solving. We make a simple promise: we will be there for your family. If you’re not satisfied, we will make the adjustments needed to meet your needs.
